A logistics company was fined £1m after an employee fell down 10m fracturing his skull, pelvic, arm, wrist, and ankle. He had fallen through an open hole in the driver’s cab of a straddle carrier, landing on the concrete floor below.

Number 9

A chemicals company was fined £1.1m. A man died following complications to his injuries, after he slipped whilst erecting a scaffolding tower.

Number 8

A metal recycling company was fined £1.2m after a worker was injured by a skip wagon, whilst walking across the site yard. Following the incident, the man sustained a fractured skull as well as his collarbone in two places but has since made a full recovery.

Number 7

The owner of Ginsters was fined £1.28m after an employee was killed by a vehicle making a delivery to the site. He was fatally crushed by the lorry as it reversed into a loading bay.

Number 6

A telecom company was fined £1.3m after an engineer was killed whilst carrying out repairs to a telephone line. He had slipped and fell into the River Aber in Wales and was swept away.

A company was fined £1.6m after a man was crushed to death when a crane collapsed causing to load to fall on top of him

Number 4

A recycling company was fined £2m after an agency worker was killed by a loading shovel. The worker died after being struck and run over by the vehicle.

Number 3

A construction company was fined £2.34m after the death of a worker. Whilst on a boat, they were affected by the turbulent water. The boat capsized and the employee was pulled underwater. Although he was rescued after 14 mins, he died upon arrival at hospital.

Number 2

A recycling company was fined £3m after an employee died and another was seriously injured whilst they were decommissioning a gas rig in the North Sea. They were on a mobile elevated work platform (MEWP) when it was struck by collapsing metalwork, throwing them 12m to the floor.

Number 1

National Grid was fined £3.2m after an employee was left with life changing injuries. Whilst working on a pylon, he received an electric shock, causing 40% burns. He required six operations in 10 days and will be a burns patient for the rest of his life.
